Apple broke (and ultimately fixed) the USB kernel extensions that are upgraded with iTunes. This has happened in the past, with iTunes 11. Looking in the Console log on the Mac I see errors with "appleusb20xhciport" and "usbagent" so I suspect a mac-side USB issue. I've rebooted both the Mac and the iPhone many times. I find that I CAN tether over WiFi and Bluetooth, just not USB.

I'm on a MacBook Pro running 10.11.6 El Capitan and the USB ports work just fine with many other devices. I tried reinstalling iTunes 12.5.1 from the Apple downloads disk image, but it did not fix.

This might have started after the iTunes 12.5 update OR after the iOS 10 update. ITunes won't even recognize the phone any more, even for charging.
